  1. Total Premium Related Revenue - Total commercial premiums and Medicare and Medicaid revenues recorded by nationwide business operations on a prepaid basis for the provision of contracted health services, net of premiums ceded for reinsurance. Premium revenues do not include fee-for-service revenues.
  2. Missouri % of "A" - Reported Missouri premium related revenues relative to reported total nationwide premium related revenues (before premiums ceded for reinsurance). The percentage, derived for HMOs with Missouri percentage of business greater than 60%, excludes premium related revenues for companies that did not meet the 60% threshold.
  3. Total Revenue - Includes: premium related revenue net of premiums ceded for reinsurance; fee-for-service revenue; investment revenue; and aggregate write-ins for other revenues.
  4. Total Medical & Hospital Expense - Includes: physician services; other professional services; outside referrals; emergency room; occupancy, depreciation and amortization, directly associated with the delivery of medical services; inpatient; incentive pool adjustments; aggregate write-ins, LESS net reinsurance claims incurred; co-payments; coordination of benefits; and subrogation.
  5. Total Administration Expense - Includes: compensation; loan interest expense; occupancy, depreciation & amortization; marketing; and aggregate write-ins for other administration expenses.
  6. Income (Loss) = Total Revenue - Total Medical & Hospital Expense - Total Administration Expense
  7. Federal Income Taxes - Federal income taxes paid for the year.
  8. Net Income (Loss) - Income adjusted for extraordinary items and federal income taxes paid.
    1. Extraordinary Items - Nonrecurring gains or losses that are unrelated to, or only incidentally related to, the ordinary activities of the entity.
